Roles & Responsibilities
- Provide compassionate, hands-on nursing care to senior residents and support their daily living needs
- Administer medications and treatments accurately and document care in the resident record
- Monitor resident condition, report changes to the RN or provider, and participate in care planning
- Assist with transfers, mobility, and activities of daily living while promoting resident dignity
- Communicate clearly and kindly with residents, families, and the interdisciplinary team
- Follow infection control practices and maintain a safe, clean environment
- Support admission, discharge, and transfer processes to ensure smooth resident transitions
Qualifications
- Current LPN license in good standing (New York state licensure required)
- CPR/BLS certification (or willingness to obtain quickly)
- Comfort working in a senior care/long-term care environment
- Strong communication and teamwork skills with a resident-centered focus
- Previous LPN experience preferred; new grads with clinical experience are welcome to apply
Education
- LPN diploma or certificate from an accredited nursing program; active NY LPN license
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and vital signs monitoring
- Knowledge of long-term care practices and resident-centered care
- Ability to prioritize tasks and handle multiple residents safely
- Familiarity with electronic charting or willingness to learn
