Roles & Responsibilities

• Provide compassionate, hands-on nursing care to senior residents and support their daily living needs
• Administer medications and treatments accurately and document care in the resident record
• Monitor resident condition, report changes to the RN or provider, and participate in care planning
• Assist with transfers, mobility, and activities of daily living while promoting resident dignity
• Communicate clearly and kindly with residents, families, and the interdisciplinary team
• Follow infection control practices and maintain a safe, clean environment
• Support admission, discharge, and transfer processes to ensure smooth resident transitions

Qualifications

• Current LPN license in good standing (New York state licensure required)
• CPR/BLS certification (or willingness to obtain quickly)
• Comfort working in a senior care/long-term care environment
• Strong communication and teamwork skills with a resident-centered focus
• Previous LPN experience preferred; new grads with clinical experience are welcome to apply

Education

• LPN diploma or certificate from an accredited nursing program; active NY LPN license

Knowledge & Skills

• Medication administration and documentation
• Basic wound care and vital signs monitoring
• Knowledge of long-term care practices and resident-centered care
• Ability to prioritize tasks and handle multiple residents safely
• Familiarity with electronic charting or willingness to learn
