Roles & Responsibilities
- Provide compassionate, direct patient care as assigned in the per diem pool
- Administer medications and treatments safely and document accurately
- Perform assessments and report changes in patient condition to the care team
- Collaborate with nurses, providers, and support staff to deliver coordinated care
- Complete required competency testing and skills checklists to maintain high standards
- Follow infection control, safety protocols, and organizational policies consistently
- Complete timely and accurate charting, including medication and care documentation
- Participate in onboarding and training activities to stay current with best practices
Qualifications
- Active Licensed Practical Nurse (LPN) license required; CT licensure verification will be performed
- Current BLS certification required
- Successful completion of Trinity specialty-specific competency testing and skills checklist
- Must pass Trinity 10-panel drug screen and required health screenings
- Required vaccinations and health items: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, and a physical/health statement
- Fit test and respirator evaluation as required by facility policies
- Professional, patient-focused attitude with strong communication and teamwork skills
Education
- Vocational Nursing diploma or equivalent qualifying education for LPN licensure
Knowledge & Skills
- Solid fundamentals in medication administration and basic nursing care
- Comfortable with patient assessment, vital signs, and documentation practices
- Knowledge of infection prevention and standard precautions
- Ability to prioritize tasks, manage time, and work independently when needed
- Good communication skills to interact with patients, families, and the care team
- Willingness to learn facility-specific systems and complete required competency checks
