Roles & Responsibilities

• Provide compassionate, direct patient care as assigned in the per diem pool
• Administer medications and treatments safely and document accurately
• Perform assessments and report changes in patient condition to the care team
• Collaborate with nurses, providers, and support staff to deliver coordinated care
• Complete required competency testing and skills checklists to maintain high standards
• Follow infection control, safety protocols, and organizational policies consistently
• Complete timely and accurate charting, including medication and care documentation
• Participate in onboarding and training activities to stay current with best practices

Qualifications

• Active Licensed Practical Nurse (LPN) license required; CT licensure verification will be performed
• Current BLS certification required
• Successful completion of Trinity specialty-specific competency testing and skills checklist
• Must pass Trinity 10-panel drug screen and required health screenings
• Required vaccinations and health items: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, Influenza vaccine, and a physical/health statement
• Fit test and respirator evaluation as required by facility policies
• Professional, patient-focused attitude with strong communication and teamwork skills

Education

• Vocational Nursing diploma or equivalent qualifying education for LPN licensure

Knowledge & Skills

• Solid fundamentals in medication administration and basic nursing care
• Comfortable with patient assessment, vital signs, and documentation practices
• Knowledge of infection prevention and standard precautions
• Ability to prioritize tasks, manage time, and work independently when needed
• Good communication skills to interact with patients, families, and the care team
• Willingness to learn facility-specific systems and complete required competency checks
