Roles & Responsibilities
- Assess residents and design individualized treatment plans that restore function and independence
- Provide hands-on therapy, gait training, balance and fall prevention interventions
- Document progress clearly and timely in the patient record to support continuity of care
- Collaborate with nurses, CNAs, and other rehab staff to coordinate resident-centered care
- Educate residents and family members on safe mobility strategies and home exercise programs
- Participate in care planning meetings and contribute measurable rehab goals
- Monitor and adjust interventions based on resident response to maximize outcomes
Qualifications
- Current Physical Therapist license or eligible for licensure in Montana
- Graduate of an accredited Physical Therapy program, DPT preferred
- Minimum experience in geriatric or skilled nursing settings preferred
- Current BLS/CPR certification
- Strong clinical reasoning, charting skills, and teamwork mindset
Education
- Doctor of Physical Therapy (DPT) from an accredited program preferred
Knowledge & Skills
- Skilled in mobility assessment, therapeutic exercise, and functional task training
- Experience with gait training, transfers, adaptive equipment, and fall prevention
- Proficient documentation skills and use of electronic medical records
- Excellent communication skills with residents, families, and care teams
- Ability to prioritize caseloads and adapt treatments to patient needs
