Roles & Responsibilities

• Assess residents and design individualized treatment plans that restore function and independence
• Provide hands-on therapy, gait training, balance and fall prevention interventions
• Document progress clearly and timely in the patient record to support continuity of care
• Collaborate with nurses, CNAs, and other rehab staff to coordinate resident-centered care
• Educate residents and family members on safe mobility strategies and home exercise programs
• Participate in care planning meetings and contribute measurable rehab goals
• Monitor and adjust interventions based on resident response to maximize outcomes

Qualifications

• Current Physical Therapist license or eligible for licensure in Montana
• Graduate of an accredited Physical Therapy program, DPT preferred
• Minimum experience in geriatric or skilled nursing settings preferred
• Current BLS/CPR certification
• Strong clinical reasoning, charting skills, and teamwork mindset

Education

• Doctor of Physical Therapy (DPT) from an accredited program preferred

Knowledge & Skills

• Skilled in mobility assessment, therapeutic exercise, and functional task training
• Experience with gait training, transfers, adaptive equipment, and fall prevention
• Proficient documentation skills and use of electronic medical records
• Excellent communication skills with residents, families, and care teams
• Ability to prioritize caseloads and adapt treatments to patient needs
