Roles & Responsibilities

• Assess residents' speech, language, cognitive-communication and swallowing abilities using evidence-based evaluations
• Develop individualized treatment plans that support meaningful functional gains
• Provide skilled speech-language therapy interventions focused on communication and dysphagia management
• Collaborate with nursing, therapy colleagues and families to coordinate care and drive positive outcomes
• Document progress clearly and timely in accordance with facility and regulatory requirements
• Educate residents and families on strategies to maintain progress and support daily communication
• Participate in care conferences and contribute clinical expertise to interdisciplinary planning

Qualifications

• ASHA Certificate of Clinical Competence (CCC) — listed as a required credential
• 1-3 years of SLP experience (experience in Skilled Nursing Facilities or geriatric therapy a strong fit)
• Active state SLP license or eligibility (confirm state requirements with the facility)
• Strong clinical reasoning, organized documentation skills and a team-oriented approach

Knowledge & Skills

• Skilled in standardized and functional SLP assessments
• Experience with dysphagia evaluation and management
• Familiarity with therapy documentation and regulatory expectations in SNF settings
• Excellent communication and interpersonal skills for resident- and family-centered care
• Ability to collaborate within an interdisciplinary team to achieve measurable outcomes
