Roles & Responsibilities

• Provide skilled nursing care and case management for patients in their homes and at the inpatient hospice center
• Perform comprehensive patient assessments and create individualized care plans using the nursing process
• Coordinate and integrate services across disciplines to ensure continuity and high-quality outcomes
• Manage symptom control and provide patient and family teaching, counseling, and referrals
• Supervise and evaluate care provided by Home Health/Hospice Aides and CNAs to ensure plan of care compliance
• Maintain accurate, timely OASIS and patient documentation in accordance with agency standards and regulations
• Participate in team scheduling, on-call rotation, quality improvement activities, and in-service training

Qualifications

• Current Registered Nurse (RN) license in the State of Montana (required)
• Minimum 2 years recent experience in acute care, home care, or hospice (preferred)
• Valid driver’s license, reliable transportation, and appropriate auto insurance required
• Clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred
• Ability to participate in rotating 12-hour shifts and on-call schedules
• Strong communication and care coordination skills for interdisciplinary collaboration

Education

• Current RN license in the State of Montana (required)
• BSN preferred

Knowledge & Skills

• Proficiency in OASIS documentation and familiarity with home health/hospice documentation standards
• Skilled in patient assessment, care planning, and symptom management
• Experience with case management and coordinating community and support services
• Ability to teach and counsel patients and families to improve outcomes and quality of life
• Comfort participating in quality improvement activities and interdisciplinary team meetings
• Knowledge of current reimbursement rules and regulations as they relate to home health
