Roles & Responsibilities

• Evaluate skilled nursing residents and create individualized PT treatment plans that drive meaningful recovery
• Deliver hands-on therapy and therapeutic exercise to improve mobility, strength, and safety
• Monitor progress, adjust plans of care, and document outcomes in the medical record with clarity and compassion
• Collaborate with nurses, OT, SLP, social work and the interdisciplinary team to provide coordinated, veteran-centered care
• Educate residents and families to maximize functional independence and long-term success
• Participate in team meetings and discharge planning to ensure smooth transitions of care

Qualifications

• PT or DPT credential (PT role noted; DPT background welcome)
• New graduates welcome — position indicates 'any, including New Grad'
• Experience or interest in Skilled Nursing Facility (SNF) practice setting preferred
• Strong communication and documentation skills to work effectively with interdisciplinary teams
• Current basic life support (BLS/CPR) certification as required by facility policies

Education

• DPT or entry-level PT degree preferred
• New graduates accepted

Knowledge & Skills

• Skilled in evaluation and treatment planning for geriatric and post-acute populations
• Proficient with mobility training, therapeutic exercise, balance and fall prevention techniques
• Good clinical documentation and outcome measurement practices
• Ability to collaborate with interdisciplinary teams and participate in discharge planning
• Comfort working in SNF workflows and prioritizing resident safety
