Roles & Responsibilities

• Evaluate residents to identify functional limitations and therapeutic goals
• Develop and implement individualized treatment plans to maximize independence
• Provide skilled therapeutic interventions and progress patients toward goals
• Document treatment plans, progress notes, and discharge recommendations clearly and timely
• Collaborate with nursing and interdisciplinary team members to coordinate comprehensive care
• Educate residents and families on safe mobility, home exercise programs and fall prevention
• Participate in care planning and contribute clinical expertise to improve outcomes

Qualifications

• PT or DPT credential (role welcomes clinicians trained as Physical Therapists)
• Any experience level accepted, including new graduates
• Current or eligible for state licensure where required (Virginia licensure processes may apply)
• Basic life support certification or willingness to obtain BLS
• Strong communication skills and ability to work collaboratively in a SNF setting

Education

• Doctor of Physical Therapy (DPT) preferred; candidates with PT training considered
• Open to new graduates

Knowledge & Skills

• Skilled in therapeutic exercise, mobility training, and ADL retraining
• Knowledge of SNF practice patterns, documentation standards, and reimbursement basics
• Ability to assess balance, gait, transfers and implement safe interventions
• Good clinical reasoning, time management, and patient/family education skills
• Comfort working within interdisciplinary teams and mentoring support staff
