Roles & Responsibilities

• Assess residents' mobility, strength, balance, and functional needs and design individualized therapy plans
• Deliver skilled physical therapy interventions to promote independence and safe mobility
• Document evaluations, treatment plans, progress notes, and discharge recommendations accurately and on time
• Collaborate with the interdisciplinary team to coordinate care and support resident goals
• Meet productivity expectations and performance standards, including the stated 90% productivity requirement
• Adhere to Continuum Rehabilitation Group policies and facility procedures to ensure safe, compliant care

Qualifications

• PT or DPT (listed as required skills: PT, DPT)
• 1-3 years of experience in physical therapy (experience range explicitly listed)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Willingness to follow CRG policies and procedures and meet productivity metrics
• Able to be present in the building to receive guaranteed hours and work in required dress code (black scrubs)

Knowledge & Skills

• Experience with SNF-level physical therapy interventions and resident-focused rehabilitation
• Strong documentation and clinical reasoning skills
• Ability to collaborate effectively with nursing and therapy team members
• Familiarity with productivity expectations and meeting performance targets
• Professionalism in appearance and adherence to facility dress code
