Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans that improve mobility and independence
Deliver evidence-based interventions to strengthen, stabilize, and restore function
Complete evaluations, progress notes, and discharge documentation in line with facility and regulatory standards
Collaborate with interdisciplinary team members to coordinate patient-centered care
Educate patients and families on therapy goals, safety precautions, and home exercise programs to promote lasting recovery
Maintain compliance with state, federal, and facility policies while advocating for high-quality rehab services

Qualifications

Active California Physical Therapist license in good standing
Graduate of an accredited Physical Therapy program
Skilled Nursing Facility or rehabilitation experience preferred
Strong communication, clinical judgment, and documentation skills
Ability to work independently and as part of a multidisciplinary team

Education

Graduate of an accredited Physical Therapy program

Knowledge & Skills

Skilled in physical therapy evaluation and treatment planning
Knowledge of rehabilitative interventions to improve mobility, strength, balance, and function
Proficient in clinical documentation and regulatory compliance
Excellent interdisciplinary collaboration and patient/family education skills
Strong clinical reasoning and time management in a skilled nursing setting
