Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatment in patients’ homes
• Develop and implement individualized care plans focused on functional independence
• Accurately document visits using required home health documentation (including OASIS where applicable)
• Coordinate care and communicate effectively with patients, families, and referring clinicians
• Travel between patient homes within assigned territories and manage time efficiently
• Maintain productivity expectations and track visit-based points to meet weekly goals
• Adhere to infection control protocols, including safe care for COVID-positive or PUI patients when required

Qualifications

• Current CA Physical Therapist license (required)
• BLS certification (required)
• Valid driver’s license, current auto registration and insurance (required)
• Current immunizations and screenings as listed: TB (annual), MMR, Varicella, Hepatitis B, TDap, annual Flu
• Successful drug screen and recent physical exam per agency policy
• Comfort with home health environment, visit-based care, and occasional travel outside assigned area
• Willingness to provide weekend coverage when needed and to see COVID+ / PUI patients

Knowledge & Skills

• Home health practices and visit-based billing concepts (OASIS experience preferred)
• Strong patient evaluation and documentation skills
• Ability to prioritize caseloads, manage travel time, and meet productivity targets
• Excellent communication and interdisciplinary coordination
• Proficiency in time management and working independently in community settings
• Competence with productivity/points tracking and meeting agency/client expectations
