Roles & Responsibilities

• Provide skilled physical therapy care in patients' homes—assess mobility, design treatment plans, and track progress
• Develop and implement individualized treatment plans that improve function and independence
• Document clinical findings and care in the medical record system (Epic preferred)
• Collaborate with nurses, aides, and interdisciplinary team members to coordinate patient-centered care
• Perform required competency testing and skills checklists to maintain clinical excellence
• Communicate clearly with patients and families—educate, motivate, and set achievable goals
• Maintain compliance with vaccination and health screening requirements to ensure patient safety

Qualifications

• Current Physical Therapy license (PT) with license verification required
• Basic Life Support (BLS) certification
• Home health experience or strong interest in home-based care preferred
• Epic experience preferred but not required
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to meet required personal health screenings and immunizations

Knowledge & Skills

• Home health assessment and care planning
• Gait, balance, transfer and mobility training
• Skilled documentation and clinical charting (Epic preferred)
• Patient and family education techniques
• Infection control and safe patient handling
• Strong communication, time management, and interdisciplinary collaboration
