Roles & Responsibilities

• Perform comprehensive OASIS assessments, recertifications, and follow-ups to ensure high-quality home health care
• Evaluate patient functional status and develop meaningful, individualized OT treatment plans
• Deliver compassionate in-home therapy visits, including routine, eval, discharge, and re-eval encounters
• Document visits thoroughly and accurately following agency and client standards
• Coordinate care with patients, families, and interdisciplinary teams, including physicians and case managers
• Maintain productivity expectations (points-based system) and adapt to guaranteed or non-guaranteed scheduling
• Provide safe care including COVID+ or PUI patients when assigned, following infection control protocols
• Travel within the assigned county territories to support patient access and continuity of care

Qualifications

• Current California Occupational Therapy license (required)
• BLS certification (required)
• Valid driver’s license, active auto insurance and vehicle registration (required)
• Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, HepB, TDap (within 10 years), annual flu, drug screen, and physical (required)
• Home health experience preferred but not required — new grads encouraged to apply
• Strong documentation skills and ability to meet productivity expectations
• Willingness to travel within assigned service areas and provide weekend coverage as needed

Knowledge & Skills

• Proficient with OASIS assessments and home health regulatory/documentation expectations
• Skilled in patient evaluation, functional assessment, and developing measurable goals
• Strong clinical reasoning for safe in-home interventions and discharge planning
• Excellent time management, organization, and documentation accuracy
• Comfortable working independently in the community and coordinating care with remote teams
• Familiarity with infection control practices and providing care to COVID+ or PUI patients
