Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and monitoring
• Perform focused assessments, document findings accurately, and escalate changes in condition
• Collaborate with interdisciplinary teams to support individualized treatment plans
• Maintain clear, timely charting and handoffs to ensure continuity of care
• Support facility policies for infection control, safety, and regulatory compliance
• Assist with admissions, discharges and coordination of patient needs within the treatment setting
• Demonstrate strong attendance and reliability to ensure consistent coverage for patients

Qualifications

• Active LVN license in good standing
• Current BLS/CPR certification or willingness to obtain upon hire (if required by facility)
• Demonstrated dependable attendance — the posting requests GOOD attendance and NO TOR
• Comfortable working the listed evening shifts and flexible to confirmed per-diem locations
• Strong communication skills and ability to work well on a clinical team
• Prior experience in medical or behavioral health settings is a plus

Education

• Vocational nursing diploma or equivalent completion of an approved LVN program

Knowledge & Skills

• Medication administration and safe documentation practices
• Patient assessment: vital signs, basic wound care, and monitoring
• Electronic charting and clinical documentation skills
• Infection control and adherence to facility policies
• Effective communication with patients, families, and care teams
• Ability to work in a structured treatment environment such as a substance abuse facility
