Roles & Responsibilities

• Provide compassionate direct nursing care to residents in a skilled nursing environment
• Administer medications and treatments accurately and safely following facility protocols
• Assess resident condition, document changes, and report concerns to the care team promptly
• Collaborate with interdisciplinary team members to develop and update individualized care plans
• Support admission, transfer and discharge processes with thorough documentation and patient education
• Mentor and support nursing aides and other staff to maintain high standards of care

Qualifications

• Current, active LPN license in Pennsylvania (required)
• Completion of an accredited practical nursing program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a long-term care/skilled nursing setting; prior experience preferred
• Clear background check and drug screen as part of onboarding
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited Practical Nursing program (Diploma or Certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and restorative nursing techniques
• Resident assessment, vital signs, and early recognition of clinical changes
• Charting and electronic medical record (EMR) familiarity
• Ability to prioritize and multitask in a fast-paced care environment
• Customer-focused approach to resident and family interactions
