Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments safely and according to facility policy
• Monitor and document resident conditions, vital signs, and care interventions
• Collaborate with interdisciplinary team members to support individualized care plans
• Assist residents with activities of daily living and mobility to promote independence
• Report changes in condition promptly and participate in shift handoffs for continuity of care
• Maintain accurate and timely electronic and paper documentation in compliance with regulations
• Adhere to infection control practices and facility safety protocols to protect residents and staff

Qualifications

• Current, active LPN or RN license (able to practice as LPN or RN in Massachusetts preferred)
• Basic Life Support (BLS/CPR) certification required or must obtain upon hire
• Prior experience in long-term care, rehab, or skilled nursing is highly valued
• Strong communication skills and the ability to work well on an interdisciplinary team
• Current immunizations and ability to pass background and health screenings per facility policy

Education

• LPN: Vocational diploma or practical nursing certificate
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) for Registered Nurse applicants

Knowledge & Skills

• Solid clinical assessment skills and ability to recognize changes in resident condition
• Medication administration and documentation proficiency
• Familiarity with electronic medical records and accurate charting practices
• Knowledge of infection prevention, wound care basics, and safety protocols
• Time management, prioritization, and teamwork in a fast-paced care environment
• Compassionate resident-centered care and excellent interpersonal skills
