Shift Type: Monday-Friday, day shift (hours vary); some weekends may be required

Roles & Responsibilities

Provide compassionate nursing care to clinic patients within scope of LPN licensure
Take and document vital signs and perform initial patient assessments
Triage patient calls effectively and coordinate next steps with providers
Process medication refills and complete necessary prior authorizations promptly
Give direction to CNAs and schedulers to ensure smooth clinic operations
Collaborate closely with providers, coordinators, clinical managers/supervisors, and RNs to deliver coordinated care
Support the interdisciplinary team to meet patient needs and improve clinic workflows

Qualifications

Current State LPN license (active and in good standing)
BLS (Basic Life Support) certification
Graduate of an accredited Nursing/Practical Nursing program
Experience with vital signs, triage, medication refills, and prior authorizations
Comfort supervising CNAs and working with scheduling staff
Strong patient communication and team collaboration skills

Education

Graduate of an accredited Practical Nursing or Nursing program (LPN)
