Roles & Responsibilities

• Administer medications and treatments safely and accurately according to care plans
• Perform timely patient assessments and monitor vital signs throughout the shift
• Document patient status, interventions, and changes in the electronic record clearly
• Assist residents with activities of daily living while promoting dignity and independence
• Collaborate with the interdisciplinary team to coordinate resident-centered care
• Provide compassionate patient and family education and support
• Follow infection control and safety protocols to maintain a healthy environment
• Respond promptly to emergencies and escalate changes in condition to RN/physician

Qualifications

• Current LPN license in New Mexico or compact/licensure eligible
• Basic Life Support (BLS) certification required
• Previous long-term care or skilled nursing experience preferred
• Strong communication and teamwork skills; dependable and patient-focused
• Ability to work the posted days and 12+ hour shifts reliably

Education

• Graduate of an accredited Practical Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and charting accuracy
• Skilled in resident assessment and recognizing clinical changes
• Familiarity with long-term care protocols and wound care basics
• Comfortable using electronic medical records for documentation
• Excellent interpersonal skills for working with residents, families, and staff
