Roles & Responsibilities

• Provide compassionate, high-quality bedside nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and on schedule
• Monitor and document patient condition, vital signs, and progress clearly and timely
• Collaborate with nurses, therapists, and care team members to create and follow individualized care plans
• Assist with ADLs (activities of daily living) and support resident independence and dignity
• Communicate effectively with residents, families, and interdisciplinary team members
• Identify changes in resident status and escalate concerns to the appropriate clinician

Qualifications

• Current Licensed Practical Nurse (LPN) or T-LPN license (state licensure required)
• Active basic life support (BLS) or CPR certification preferred or obtainable upon hire
• Minimum clinical experience caring for older adults or in a long-term care/skilled nursing environment preferred
• Ability to pass background check, drug screen, and meet facility onboarding requirements
• Flexible, reliable, and able to thrive in a travel assignment environment

Education

• LPN diploma or vocational nursing certificate
• High school diploma or GED

Knowledge & Skills

• Skilled in medication administration and documentation
• Comfortable with wound care, ADL support, and basic restorative nursing
• Strong clinical assessment and vital sign interpretation skills
• Clear, compassionate communication with residents and families
• Familiarity with electronic health records or willingness to learn quickly
• Team-oriented approach and good time management
