Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: As scheduled (per diem), 7:00 AM - 7:30 PM Day shift

Hours per Week: 38

Candidate Pay Rate: $21 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents including assistance with activities of daily living
Monitor and record vital signs, intake/output, and any changes in condition accurately
Support safe mobility and transfers, using proper body mechanics and assistive devices
Communicate observations and care concerns clearly to nurses and the care team
Maintain a clean, safe, and comfortable environment for residents
Assist with meal service and feeding as needed, respecting dietary needs and preferences

Qualifications

Active Licensed Nursing Assistant (LNA) certification
Current Basic Life Support (BLS/CPR) preferred or willingness to obtain
Comfortable working 12+ hour day shifts as scheduled
Ability to pass background check and meet facility health requirements
Strong interpersonal skills and a patient-centered attitude

Education

High school diploma or equivalent
Completion of an approved LNA training program

Knowledge & Skills

Skilled in basic patient care and assistance with ADLs
Familiarity with vital sign measurement and documentation
Good charting and communication with interdisciplinary teams
Knowledge of infection control and safety procedures
Ability to follow care plans and report changes promptly
Team player who supports residents and fellow staff
