Contract Type: Per-Diem

Shift Type: Per diem (days vary), 7:00 AM - 7:30 PM Day shift

Hours per Week: 36

Candidate Pay Rate: $21 per hour

Roles & Responsibilities

Provide compassionate, hands-on personal care to residents with dignity and respect
Assist with activities of daily living including bathing, dressing, toileting, and mobility
Monitor and report changes in resident condition to nursing staff promptly
Document care and activities accurately and timely in the resident record
Support safe transfers and fall-prevention measures using proper techniques
Work collaboratively with the care team to deliver consistent, person-centered care

Qualifications

Current Licensed Nursing Assistant (LNA) credential (VT or NH eligibility noted)
Active state registration as required for assignment
Comfortable working 12.5-hour day shifts (7:00 AM - 7:30 PM) in a per-diem model
Strong communication skills and a positive, patient-first attitude
Reliable, punctual, and ready to step in as part of a collaborative team

Education

Completion of an approved LNA program
High school diploma or equivalent preferred

Knowledge & Skills

Patient care fundamentals and safe transfer techniques
Basic infection control and hygiene practices
Accurate documentation and time management
Ability to follow care plans and delegations from licensed nurses
Clear verbal communication with residents, families, and team members
Compassionate approach to supporting residents with diverse needs
