Roles & Responsibilities
- Provide skilled physical therapy evaluations and develop individualized treatment plans
- Deliver hands-on interventions and therapeutic exercise to improve patient function
- Document patient progress, treatment plans and outcomes in the facility EMR
- Communicate and collaborate with nurses, aides and interdisciplinary team members
- Educate patients and families on safe mobility, fall prevention and home exercise programs
- Ensure compliance with facility policies, professional standards and safety protocols
- Participate in discharge planning to support smooth transitions of care
Qualifications
- Current Physical Therapist license (MS license preferred or ability to obtain state licensure)
- Active CPR/BLS certification or willingness to obtain prior to start
- Experience in skilled nursing and rehab settings is highly valued
- Strong clinical assessment, documentation and time-management skills
- Ability to float to nearby facilities when needed (Winona Manor noted)
- Professional, reliable and patient-centered approach to care
Education
- Doctor of Physical Therapy (DPT) or equivalent entry-level PT degree preferred
- Associations with state licensure requirements
Knowledge & Skills
- Skilled in therapeutic exercise, manual therapy and gait training
- Comfortable with mobility devices and ADL training
- Proficient documentation and electronic medical record use
- Excellent communication and teamwork skills
- Knowledge of safety standards, infection control and discharge planning
