Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on interventions and therapeutic exercise to improve patient function
• Document patient progress, treatment plans and outcomes in the facility EMR
• Communicate and collaborate with nurses, aides and interdisciplinary team members
• Educate patients and families on safe mobility, fall prevention and home exercise programs
• Ensure compliance with facility policies, professional standards and safety protocols
• Participate in discharge planning to support smooth transitions of care

Qualifications

• Current Physical Therapist license (MS license preferred or ability to obtain state licensure)
• Active CPR/BLS certification or willingness to obtain prior to start
• Experience in skilled nursing and rehab settings is highly valued
• Strong clinical assessment, documentation and time-management skills
• Ability to float to nearby facilities when needed (Winona Manor noted)
• Professional, reliable and patient-centered approach to care

Education

• Doctor of Physical Therapy (DPT) or equivalent entry-level PT degree preferred
• Associations with state licensure requirements

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and gait training
• Comfortable with mobility devices and ADL training
• Proficient documentation and electronic medical record use
• Excellent communication and teamwork skills
• Knowledge of safety standards, infection control and discharge planning
