Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehab and skilled care setting
• Administer medications and treatments in accordance with facility policy and scope of practice
• Assess and monitor patient status, documenting changes and communicating concerns promptly
• Support and assist interdisciplinary rehab teams to help patients reach their goals
• Perform thorough and timely charting in the electronic medical record
• Maintain safety and infection control standards while promoting resident dignity
• Mentor and collaborate with nursing assistants and other care team members to ensure quality care

Qualifications

• Active LPN license in the state of practice (New Mexico) or ability to obtain prior to start
• Current Basic Life Support (BLS/CPR) certification
• Previous experience in long-term care, skilled nursing, or rehab preferred
• Ability to work 12+ hour night shifts and adapt quickly to a new facility
• Willingness to complete required facility screenings and onboarding paperwork

Education

• Vocational nursing diploma or equivalent completion of an accredited LPN program

Knowledge & Skills

• Strong medication administration and documentation skills
• Patient assessment and vital sign monitoring
• Familiarity with rehab nursing practices and skilled nursing care
• Comfortable using electronic health records and charting systems
• Excellent communication, teamwork, and time management skills
• Ability to prioritize tasks in a fast-paced night shift environment
