Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in the rehab and skilled care setting
- Administer medications and treatments in accordance with facility policy and scope of practice
- Assess and monitor patient status, documenting changes and communicating concerns promptly
- Support and assist interdisciplinary rehab teams to help patients reach their goals
- Perform thorough and timely charting in the electronic medical record
- Maintain safety and infection control standards while promoting resident dignity
- Mentor and collaborate with nursing assistants and other care team members to ensure quality care
Qualifications
- Active LPN license in the state of practice (New Mexico) or ability to obtain prior to start
- Current Basic Life Support (BLS/CPR) certification
- Previous experience in long-term care, skilled nursing, or rehab preferred
- Ability to work 12+ hour night shifts and adapt quickly to a new facility
- Willingness to complete required facility screenings and onboarding paperwork
Education
- Vocational nursing diploma or equivalent completion of an accredited LPN program
Knowledge & Skills
- Strong medication administration and documentation skills
- Patient assessment and vital sign monitoring
- Familiarity with rehab nursing practices and skilled nursing care
- Comfortable using electronic health records and charting systems
- Excellent communication, teamwork, and time management skills
- Ability to prioritize tasks in a fast-paced night shift environment
