Roles & Responsibilities
- Provide compassionate day-to-day nursing care to patients in the Lebanon center
- Administer medications and treatments accurately and safely
- Assess and monitor patient conditions, documenting changes clearly in the chart
- Collaborate with nurses, providers, and support staff to coordinate patient care
- Educate patients and families about care plans and home health needs
- Maintain infection control standards and promote a safe care environment
- Respond quickly and professionally to urgent situations and changes in patient status
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing (required)
- Basic Life Support (BLS/CPR) certification (preferred or required per facility)
- Prior experience in long-term care, rehab, or similar clinical setting is a plus
- Strong communication skills and ability to work well in a team
- Up-to-date immunizations and ability to complete facility onboarding
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Medication administration and accurate charting
- Patient assessment and basic vital signs interpretation
- Wound care and basic nursing procedures
- Electronic medical records (EMR) documentation
- Infection prevention and safety protocols
- Professional bedside manner and time management
