Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: Per diem (days vary), 7:00 AM - 7:30 PM Day shift

Hours per Week: 36

Candidate Pay Rate: $36 per hour

Roles & Responsibilities

Provide compassionate day-to-day nursing care to patients in the Lebanon center
Administer medications and treatments accurately and safely
Assess and monitor patient conditions, documenting changes clearly in the chart
Collaborate with nurses, providers, and support staff to coordinate patient care
Educate patients and families about care plans and home health needs
Maintain infection control standards and promote a safe care environment
Respond quickly and professionally to urgent situations and changes in patient status

Qualifications

Current Licensed Practical Nurse (LPN) license in good standing (required)
Basic Life Support (BLS/CPR) certification (preferred or required per facility)
Prior experience in long-term care, rehab, or similar clinical setting is a plus
Strong communication skills and ability to work well in a team
Up-to-date immunizations and ability to complete facility onboarding

Education

Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

Medication administration and accurate charting
Patient assessment and basic vital signs interpretation
Wound care and basic nursing procedures
Electronic medical records (EMR) documentation
Infection prevention and safety protocols
Professional bedside manner and time management
