Shift Type: Per diem / varies (facility-driven schedule), 7:00 PM - 7:30 AM NOC (overnight)

Roles & Responsibilities
- Provide compassionate personal care and assist residents with activities of daily living
- Support safe transfers, ambulation, and mobility with proper body mechanics
- Monitor and record vital signs and changes in patient condition accurately
- Maintain clear, timely documentation in resident charts and communicate observations to the nursing team
- Follow infection control protocols and ensure a clean, safe environment
- Engage residents with dignity and respect, supporting emotional and social needs
- Collaborate with colleagues to deliver consistent, high-quality care
Qualifications
- Current LNA / Licensed Nursing Assistant certification (NH required; VT eligibility helpful)
- Completion of a state-approved nursing assistant program
- CPR/BLS certification preferred or willing to obtain
- Ability to pass background checks and meet facility vaccination/immunization requirements
- Comfort working overnight shifts and providing hands-on resident care
- Reliable transportation and availability for per diem scheduling
Education
- High school diploma or GED
- Completion of a state-approved nursing assistant program and active LNA certification
Knowledge & Skills
- Familiarity with activities of daily living (ADLs) assistance and patient hygiene
- Basic vital signs measurement and reporting
- Strong communication and interpersonal skills with residents and multidisciplinary teams
- Working knowledge of infection control and safety procedures
- Dependable time management and the ability to prioritize resident needs
- Comfortable using electronic charting or paper documentation systems
