Roles & Responsibilities

• Provide compassionate personal care and assist residents with activities of daily living
• Support safe transfers, ambulation, and mobility with proper body mechanics
• Monitor and record vital signs and changes in patient condition accurately
• Maintain clear, timely documentation in resident charts and communicate observations to the nursing team
• Follow infection control protocols and ensure a clean, safe environment
• Engage residents with dignity and respect, supporting emotional and social needs
• Collaborate with colleagues to deliver consistent, high-quality care

Qualifications

• Current LNA / Licensed Nursing Assistant certification (NH required; VT eligibility helpful)
• Completion of a state-approved nursing assistant program
• CPR/BLS certification preferred or willing to obtain
• Ability to pass background checks and meet facility vaccination/immunization requirements
• Comfort working overnight shifts and providing hands-on resident care
• Reliable transportation and availability for per diem scheduling

Education

• High school diploma or GED
• Completion of a state-approved nursing assistant program and active LNA certification

Knowledge & Skills

• Familiarity with activities of daily living (ADLs) assistance and patient hygiene
• Basic vital signs measurement and reporting
• Strong communication and interpersonal skills with residents and multidisciplinary teams
• Working knowledge of infection control and safety procedures
• Dependable time management and the ability to prioritize resident needs
• Comfortable using electronic charting or paper documentation systems
