Roles & Responsibilities

• Provide skilled pediatric physical therapy services across home, preschool and school-based settings
• Perform evaluations and develop individualized treatment plans focused on functional progress
• Deliver early intervention home visits for birth-to-3 children with family-centered goals
• Collaborate closely with educators, caregivers and interdisciplinary teams to coordinate care
• Document progress clearly and timely to support school records and IFSP/IEP goals
• Coach families and school staff on carryover activities and adaptive strategies
• Participate in student-centered planning and support smooth transitions between settings

Qualifications

• Current Oregon Physical Therapy License (required)
• Experience in pediatric physical therapy, including early intervention (birth-3) and preschool (3-5)
• Comfortable providing home visits and school-based therapy services
• Experience with school-based therapy settings preferred
• Strong communication skills for collaborating with families, teachers and multidisciplinary teams

• School-based therapy processes and documentation expectations
• Family-centered care and coaching for home programs
• Ability to adapt interventions across home, preschool and school environments
