Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents, focusing on dignity and comfort
- Administer medications and treatments safely and accurately following facility protocols
- Assess residents’ conditions, monitor vital signs, and report changes promptly to the care team
- Document assessments, interventions, and outcomes clearly in the resident record
- Collaborate with RNs, therapists, and support staff to deliver coordinated care plans
- Assist with ADLs (bathing, dressing, mobility) while promoting independence whenever possible
- Support discharge planning and communicate effectively with families and care partners
Qualifications
- Active LPN license in Massachusetts required (RN candidates previously considered per facility needs)
- Current BLS/CPR certification required
- Recent long-term care or rehabilitation experience preferred but new grads with strong skills are welcome
- Excellent communication skills and a team-first attitude
- Ability to work per diem shifts, adapt to changing schedules, and support residents across the care continuum
Education
- Completed accredited LPN (Vocational Nursing) program; equivalent training considered
Knowledge & Skills
- Strong clinical assessment and medication administration skills
- Familiarity with wound care, catheter care, and common long-term care procedures
- Comfortable using electronic health records and documenting care accurately
- Ability to prioritize tasks, multi-task in a busy environment, and escalate concerns appropriately
- Person-centered approach with strong interpersonal and problem-solving skills
