Roles & Responsibilities
- Provide compassionate bedside care and daily living assistance to residents
- Administer medications and treatments accurately and document per policy
- Perform routine assessments and report changes in patient condition
- Collaborate with the interdisciplinary team to deliver individualized care plans
- Maintain accurate, timely charting in the electronic medical record
- Support admission, transfer, and discharge processes to ensure smooth transitions
- Advocate for resident comfort, safety, and dignity at all times
Qualifications
- Active Massachusetts LPN license (or RN license if applying as an RN)
- Current BLS/CPR certification
- Experience in long-term care, skilled nursing, or rehab settings preferred
- Strong communication skills and ability to work well within a care team
- Proof of immunizations and ability to pass facility-required screenings
Education
- LPN: Completed practical nursing diploma or certificate program
- RN applicants: Associate or Bachelor of Science in Nursing (if applicable)
Knowledge & Skills
- Clinical assessment and vital signs monitoring
- Medication administration and documentation
- Wound care basics and infection control practices
- Familiarity with electronic medical records and charting
- Time management and prioritization in a fast-paced environment
- Compassionate resident-centered care and strong interpersonal skills
