Roles & Responsibilities

• Provide compassionate direct resident care including vitals, basic assessments, and daily living assistance
• Administer medications and treatments accurately and safely according to facility policies
• Document resident status, interventions, and shift reports clearly in the medical record
• Collaborate with the interdisciplinary team to support individualized care plans
• Assist with wound care, dressing changes, and monitoring for changes in condition
• Participate in admission, transfer, and discharge processes to ensure continuity of care
• Support a positive, respectful environment that promotes resident dignity and independence

Qualifications

• Active LPN or RN license in Massachusetts required
• Current BLS/CPR certification required
• Ability to pass background check and fulfill facility onboarding requirements
• Preferred: prior experience in long-term care, rehab, or skilled nursing settings
• Reliable clinical judgment, strong communication skills, and a team-oriented attitude
• Up-to-date immunizations and TB screening per facility policy

Knowledge & Skills

• Patient assessment and monitoring of vital signs
• Medication administration and documentation best practices
• Basic wound care and infection control principles
• Familiarity with electronic health record charting and shift reporting
• Strong interpersonal skills for working with residents, families, and the care team
• Ability to prioritize tasks in a fast-paced care environment
