Roles & Responsibilities

• Provide safe, compassionate nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments according to physician orders and facility policy
• Perform timely and accurate patient assessments and document findings in the medical record
• Collaborate with the interdisciplinary team to support individualized care plans
• Assist residents with activities of daily living and mobility as needed
• Monitor vital signs and report changes in condition promptly to the care team
• Adhere to infection control protocols and maintain a safe environment for residents

Qualifications

• Current Massachusetts LPN or RN license in good standing (LPN preferred for this posting)
• Basic Life Support (BLS) / CPR certification required or ability to obtain promptly
• Relevant experience in long-term care, skilled nursing, or rehabilitation preferred
• Ability to pass required background checks and health screenings
• Reliable, professional, and able to work independently on per-diem assignments
• Strong communication skills and a team-oriented attitude

Education

• Graduate of an accredited LPN program (or RN program if applying as RN)
• High school diploma or GED required; post-secondary nursing education required

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and care planning
• Basic wound care and restorative nursing support
• Familiarity with electronic medical records and charting
• Infection prevention and control principles
• Time management and prioritization in a busy clinical setting
• Effective teamwork and clear communication with patients and staff
