Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans focused on restoring mobility and independence
• Deliver evidence-based therapeutic interventions such as therapeutic exercise, gait training, and balance work
• Collaborate with nursing and interdisciplinary teams to coordinate safe, patient-centered care
• Document progress and outcomes clearly and timely to support continuity of care
• Educate patients and families on home programs and safe functional strategies
• Participate in discharge planning and make appropriate referrals to ensure optimal transitions

Qualifications

• Doctor of Physical Therapy (DPT) preferred — new grads welcome
• Current Illinois Physical Therapist license or eligible to obtain licensure
• Experience or interest in skilled nursing facility (SNF) care
• Strong communication and teamwork skills to collaborate with interdisciplinary staff
• Basic life support (BLS) preferred or willingness to obtain

Education

• Doctor of Physical Therapy (DPT) preferred; entry-level PT credentials accepted

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and transfers
• Familiarity with documentation standards and SNF regulatory expectations
• Ability to design safe, progressive home and facility-based programs
• Good clinical reasoning and problem-solving in a fast-paced setting
• Patient- and family-centered education and motivational coaching
