Roles & Responsibilities
- Evaluate residents through thorough assessments and develop individualized treatment plans
- Deliver hands-on skilled therapy focused on mobility, balance, strength, and functional independence
- Document progress clearly and timely in the medical record to support continuity of care
- Collaborate with nursing, OT, SLP, and social work to coordinate discharge plans and goals
- Educate residents and families on home programs and safe mobility strategies
- Participate in therapy team meetings and contribute to quality improvement initiatives
Qualifications
- PT or DPT credential (PT/DPT) indicated as core skills
- Any experience level welcome, including new graduates (years of experience: any, including New Grad)
- Comfort working in a skilled nursing facility (SNF) setting
- Strong communication and teamwork skills to coordinate resident care
Education
- Doctor of Physical Therapy (DPT) or equivalent physical therapist education; new grads welcome
Knowledge & Skills
- Geriatric rehabilitation principles and SNF-focused care
- Patient evaluation and skilled therapeutic intervention planning
- Functional mobility training, transfers, gait training, and fall prevention
- Clear clinical documentation and goal-oriented progress notes
- Ability to collaborate effectively with interdisciplinary teams
- Compassionate patient and family education skills
