Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day); possible start times include 7:00 AM, 7:30 AM, 8:00 AM or 8:30 AM; weekend coverage required every other weekend (or 4 weekend days in a 4-week schedule). Shift type: day.
Hours per Week: 40
Candidate Pay Rate: $66 per hour

Roles & Responsibilities
- Coordinate comprehensive discharge planning to ensure safe transitions of care
- Perform needs assessments and arrange DME, home health, and other post-acute services
- Conduct utilization review and apply Milliman guidelines to support appropriate care
- Document thoroughly and efficiently in Epic to maintain accurate patient records
- Collaborate with interdisciplinary teams including social work, PT/OT, pharmacy, and providers
- Develop and implement individualized plans of care that prioritize patient goals
- Educate patients and families about follow-up care, resources, and community supports
- Support department coverage by floating within case management services as needed
Qualifications
- AK RN license in hand required for auto-offer (screen for pending license)
- Minimum 1 year acute care nursing experience
- BLS required; ACM (ACM-RN) certification preferred
- Epic EMR experience required (hardstop) — strong charting skills expected
- Experience with case management or utilization review and discharge planning
- Ability to work business hours and meet weekend coverage requirements
- Locals not accepted: permanent address must be at least 50 miles from the facility
Knowledge & Skills
- Proficiency with Epic charting and documentation workflows
- Knowledge of care coordination, discharge planning, and utilization review
- Familiarity with Milliman guidelines and CMS/HIPAA regulations
- Comfort collaborating with interdisciplinary teams and community resources
- Ability to conduct needs assessments and arrange appropriate DME and services
- Working knowledge of Microsoft Office and Microsoft Teams preferred
- Experience across patient age groups from neonates to geriatrics is a plus
