Roles & Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans that target mobility, strength, balance, and function
Deliver skilled therapy services to help patients regain independence and meet rehab goals
Monitor and document patient progress accurately and in a timely manner to support care continuity
Collaborate with interdisciplinary healthcare teams to coordinate care and optimize outcomes
Educate patients and caregivers on therapy goals and effective home exercise programs to support long-term recovery

Qualifications

Active California Physical Therapist license (required)
Experience in a Skilled Nursing or rehabilitation setting preferred
Strong clinical judgment and excellent communication skills
Ability to work independently while also contributing effectively as part of a team

Knowledge & Skills

Physical Therapy principles and patient-centered care
Patient evaluation and individualized treatment planning
Skilled therapeutic interventions to improve mobility, strength, balance, and function
Accurate clinical documentation and progress monitoring
Interdisciplinary collaboration and patient/caregiver education
