Roles & Responsibilities

• Perform comprehensive patient evaluations and create individualized treatment plans that make a real difference
• Implement therapeutic interventions to improve strength, mobility, balance, and independence
• Monitor patient progress and adjust plans to ensure measurable improvements
• Educate patients and caregivers on home exercise programs and safety strategies for lasting outcomes
• Maintain accurate, timely documentation in line with facility and regulatory standards
• Collaborate with interdisciplinary team members to deliver coordinated, patient-centered care

Qualifications

• Active California Physical Therapist (PT) license
• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Current CPR/BLS certification
• Minimum 1 year of PT experience preferred (SNF, outpatient, or acute care experience is a plus)
• Strong communication and interpersonal skills

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy acceptable where applicable

Knowledge & Skills

• Patient evaluation and functional assessment skills
• Individualized treatment planning and implementation
• Therapeutic exercise, balance and mobility training techniques
• Clinical documentation and regulatory compliance
• Interdisciplinary collaboration and patient/caregiver education
• Clear communication and strong interpersonal rapport
