Roles & Responsibilities

• Evaluate patient function using formal and informal assessments to create a clear picture of needs.
• Develop individualized, goal-driven treatment plans and update the plan of care as patients progress.
• Deliver evidence-based treatment interventions to restore or enhance daily functioning.
• Educate patients, families and caregivers on conditions, prognosis and home programs to support lasting gains.
• Supervise Occupational Therapist Assistants, home health aides and students with clear guidance and mentorship.
• Document patient response and outcomes thoroughly and in compliance with affiliate policies and regulations.
• Recommend and coordinate adaptive/supportive equipment to improve patient independence.
• Serve as case manager for assigned patients, coordinating care across the interdisciplinary team.
• Participate in team conferences, promote program development, and champion process improvement initiatives.

Qualifications

• OTR — Occupational Therapist Registered (required)
• NBCOT certification (required)
• BLS (Basic Life Support) — to be completed within one month of hire if not current
• Bachelor's degree (required); degree in occupational therapy or related field preferred
• More than 1 year of clinical experience preferred — new graduates are welcome to apply
• Ability to work independently, manage time well, and demonstrate strong problem-solving skills
• Ability to use personal protective equipment and work where exposure to communicable diseases may occur
• Mandatory OSHA training completed annually per Exposure Control Plan

Education

• Bachelor's Degree (required)

Knowledge & Skills

• Skilled in assessment, treatment planning, intervention and evaluation
• Strong documentation practices and familiarity with billing-related documentation
• Patient and family education techniques to promote home program adherence
• Experience supervising OT assistants and students
• Case management and care coordination abilities
• Comfortable with Microsoft Windows and technology used in clinical documentation
• Knowledge of home health processes and community-based care (preferred)
• Familiarity with OSHA requirements and safe handling of bloodborne pathogens
