Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients with attention and respect
• Administer medications and treatments accurately and document care thoroughly
• Perform focused patient assessments and report changes in condition to the care team
• Collaborate with nurses, therapists and support staff to create and follow individualized care plans
• Assist with admissions, discharges and transfers in a calm, organized manner
• Contribute to a positive, safe environment by following facility policies and infection control practices
• Support patient and family education in a clear, empathetic way to improve outcomes

Qualifications

• Current Licensed Practical Nurse (LPN) license appropriate for Pennsylvania practice
• Basic Life Support (BLS) certification required or obtained upon hire
• Comfortable with medication administration, documentation and basic clinical procedures
• Strong communication skills and a team-first attitude
• Ability to pass background check and required health screenings

Education

• Completion of an accredited practical nursing program and active LPN licensure

Knowledge & Skills

• Skilled in clinical assessment, medication administration and wound care basics
• Familiarity with accurate charting and documentation practices
• Ability to prioritize tasks in a busy care environment and remain calm under pressure
• Good interpersonal skills for working with residents, families and multidisciplinary teams
• Commitment to resident-centered care and safety practices
