Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents at the rehab & nursing facility
- Administer medications and treatments safely and in line with facility policy
- Perform thorough patient assessments and promptly report changes in condition
- Collaborate with nurses, therapists, and interdisciplinary team members to create meaningful care plans
- Accurately document care, observations, and interventions in the patient record
- Assist with admissions, discharges, transfers, and ensure smooth transitions of care
- Support infection control practices and maintain a clean, safe environment for residents
Qualifications
- Current Licensed Practical Nurse (LPN) license — Massachusetts license preferred
- BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working 12-hour day shifts (8:00 AM - 8:00 PM)
- Previous experience in long-term care, rehab, or skilled nursing is a plus
- Strong communication skills and a team-first attitude
Education
- Completion of an accredited practical nursing program
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and basic IV/PO medication knowledge
- Resident assessment and documentation best practices
- Familiarity with rehab and long-term care workflows
- Basic wound care and ADL assistance
- Electronic health record (EHR) navigation and accurate charting
- Infection control and safety procedures
