Roles & Responsibilities

• Provide compassionate direct patient care to residents in a long-term care setting
• Administer medications and treatments safely and accurately
• Perform assessments, monitor changes in condition, and report concerns promptly
• Document care thoroughly and maintain accurate patient records
• Collaborate with RN colleagues, therapists and interdisciplinary team members to support resident goals
• Educate residents and families about care plans and promote comfort and dignity
• Follow infection control and safety protocols to maintain a clean, safe environment

Qualifications

• Active LPN license in Massachusetts (required) — or currently licensed RN and interested candidates should inquire
• Current BLS/CPR certification (or willingness to obtain)
• Previous experience in long-term care or rehab preferred but new grads with strong clinical skills are welcome
• Excellent communication and teamwork skills with a patient-centered approach
• Ability to perform clinical tasks and document care in an electronic charting system

Education

• Licensed Practical Nurse: Completion of an accredited Practical Nursing program
• Registered Nurse candidates: ADN or BSN from an accredited nursing program (if applicable)

Knowledge & Skills

• Strong knowledge of medication administration and wound care basics
• Clinical assessment and ability to recognize changes in condition
• Comfortable with charting, care plans, and basic EMR navigation
• Understanding of infection control practices and safety standards
• Compassionate bedside manner and effective communication with residents and families
