Roles & Responsibilities

• Assess patients with movement dysfunction or pain and develop individualized treatment plans
• Deliver therapeutic treatments to improve mobility, reduce pain, and enhance independence
• Implement fall risk reduction strategies and other safety interventions
• Support discharge planning to help patients transition safely back to the community
• Document evaluations, progress notes, daily notes, discharges, and related communications thoroughly
• Manage interactions with insurance for approvals/denials and maintain accurate records
• Communicate professionally with patients, families, providers, rehab staff, and supervisors
• Assist with maintaining treatment areas, including light cleaning and laundry tasks as needed
• Perform other duties as requested to support excellent patient outcomes

Qualifications

• Demonstrated experience in physical therapy focusing on pain management, mobility improvement, and rehabilitation
• Proven ability to perform patient assessments and create effective treatment plans
• Strong documentation skills for evaluations, progress notes, discharges, and insurance correspondence
• Clear, professional communication skills for collaborating with patients, providers, and payers
• Comfortable supporting treatment room upkeep and basic clinical housekeeping tasks
• Commitment to patient safety and fall risk reduction strategies

Knowledge & Skills

• Patient assessment and therapeutic treatment techniques
• Rehabilitation principles and functional mobility training
• Pain management strategies and fall risk assessment
• Discharge planning processes and coordination of care
• Thorough clinical documentation practices and familiarity with insurance correspondence
