Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
• Administer medications and treatments accurately and document promptly
• Perform timely assessments and report changes in condition to the care team
• Collaborate with nurses, therapists and families to create and follow individualized care plans
• Maintain clear, accurate clinical documentation and charting
• Support admissions, discharges and transfers with professionalism and attention to detail
• Participate in safety, infection control and quality improvement initiatives

Qualifications

• Current LPN license in Massachusetts (or eligible to obtain state licensure)
• Previous experience in long-term care, rehab, or skilled nursing preferred
• Current Basic Life Support (BLS) certification (or willing to obtain)
• Comfortable with medication administration, wound care basics and resident assessments
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)
• For RN candidates (if applying), an Associate or Bachelor of Science in Nursing is expected

Knowledge & Skills

• Clinical skills in medication administration and basic wound care
• Resident assessment and reporting changes in condition
• Accurate electronic and paper charting/documentation
• Infection control best practices and safe patient handling
• Team collaboration, time management and prioritization during busy shifts
