Roles & Responsibilities
- Provide compassionate, high-quality bedside nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document per facility policy
- Perform thorough patient assessments and communicate clinical changes to the care team
- Collaborate with nurses, therapists, and physicians to develop and follow individualized care plans
- Assist with wound care, IV management, and other skilled nursing procedures as appropriate
- Educate residents and families about care plans, medications, and safe discharge goals
- Maintain clear, timely documentation in the medical record and ensure compliance with regulations
- Support a safe, respectful environment and mentor junior staff when needed
Qualifications
- Current Massachusetts LPN license required; RN candidates considered for appropriate roles
- Current BLS/CPR certification required
- Experience in skilled nursing, rehab, or long-term care preferred
- Strong clinical assessment and medication administration skills
- Ability to work nights and collaborate effectively with a multidisciplinary team
- Excellent communication and documentation skills
Education
- LPN: Graduate of an accredited practical nursing program
- RN candidates: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) as applicable
Knowledge & Skills
- Familiarity with medication administration and common nursing procedures
- Knowledge of infection control practices and patient safety protocols
- Comfortable using electronic health records for documentation
- Strong prioritization, time management, and delegation skills
- Good clinical judgment and the ability to escalate concerns appropriately
