Roles & Responsibilities

• Administer medications and treatments accurately and on schedule, following provider orders
• Monitor and document patient vital signs, condition changes, and responses to treatment
• Provide compassionate hands-on care and assist residents with activities of daily living
• Collaborate with the interdisciplinary care team to create and update individualized care plans
• Perform wound care, catheter management, and other skilled nursing procedures as required
• Educate residents and families about care plans, medications, and self-care strategies
• Maintain accurate, timely documentation in the patient record and ensure regulatory compliance
• Promote a safe, respectful, and healing environment for residents and team members

Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts
• Current BLS (Basic Life Support) certification from an accredited provider
• Clinical experience in long-term care, rehab, or skilled nursing preferred
• Strong communication and teamwork skills with a resident-centered approach
• Ability to perform required physical duties of the role and meet facility standards

Education

• Practical Nursing diploma or equivalent for LPN licensure

Knowledge & Skills

• Medication administration and basic clinical procedures
• Familiarity with long-term care and rehabilitation nursing practices
• Accurate clinical documentation and charting skills
• Patient assessment and vital sign monitoring
• Effective communication with residents, families, and interdisciplinary teams
• Time management and ability to prioritize in a busy clinical setting
