Roles & Responsibilities

• Assess patients’ speech, language, cognitive-communication, and swallowing function and create individualized treatment plans
• Deliver evidence-based, skilled therapy interventions and modify approaches based on patient progress
• Collaborate with nurses, therapists, families, and caregivers to coordinate care and maximize outcomes
• Document evaluations, progress notes, and discharge summaries accurately and in a timely manner
• Educate staff and family members on communication strategies and safe swallowing techniques
• Participate in interdisciplinary team meetings and contribute to resident-centered care planning
• Ensure compliance with facility policies, state licensure requirements, and documentation standards

Qualifications

• Licensed Speech-Language Pathologist (SLP) in Pennsylvania (required)
• Master’s degree in Speech-Language Pathology (or equivalent) required
• ASHA CCC-SLP preferred
• BLS/CPR certification or willingness to obtain
• Experience in skilled nursing/rehab settings preferred; new grads with strong clinical skills are welcome
• Ability to complete facility onboarding and meet credentialing timelines

Education

• Master’s degree in Speech-Language Pathology (ASHA-accredited program preferred)

Knowledge & Skills

• Skilled in clinical evaluations, treatment planning, and dysphagia management
• Strong knowledge of evidence-based SLP interventions across the lifespan
• Proficient with documentation in EHRs and timely clinical charting
• Excellent communication and patient/family education skills
• Collaborative team player with strong interdisciplinary coordination abilities
• Good time management, organization, and caseload prioritization skills
