Roles & Responsibilities

Assist the Occupational Therapist in implementing individualized treatment plans to improve patient outcomes
Provide therapy services focused on ADLs and functional mobility to help patients regain independence
Educate patients and families on exercises, safety, and adaptive techniques to support long-term progress
Collaborate with the interdisciplinary rehab team to coordinate care and optimize outcomes
Complete accurate and timely documentation to ensure continuity of care and regulatory compliance

Qualifications

Active California COTA license (required)
Certified Occupational Therapy Assistant credential
SNF or rehabilitation experience preferred
Strong communication and clinical skills
Ability to work independently and as part of a collaborative team

Knowledge & Skills

Patient-centered therapeutic techniques for activities of daily living and mobility
Clinical documentation and timely charting
Interdisciplinary team collaboration and communication
Educating patients and families on adaptive strategies and safety
Experience working in a skilled nursing facility (SNF) or rehab setting
