Roles & Responsibilities
- Assist the Occupational Therapist in implementing individualized treatment plans to improve patient outcomes
- Provide therapy services focused on ADLs and functional mobility to help patients regain independence
- Educate patients and families on exercises, safety, and adaptive techniques to support long-term progress
- Collaborate with the interdisciplinary rehab team to coordinate care and optimize outcomes
- Complete accurate and timely documentation to ensure continuity of care and regulatory compliance
Qualifications
- Active California COTA license (required)
- Certified Occupational Therapy Assistant credential
- SNF or rehabilitation experience preferred
- Strong communication and clinical skills
- Ability to work independently and as part of a collaborative team
Knowledge & Skills
- Patient-centered therapeutic techniques for activities of daily living and mobility
- Clinical documentation and timely charting
- Interdisciplinary team collaboration and communication
- Educating patients and families on adaptive strategies and safety
- Experience working in a skilled nursing facility (SNF) or rehab setting
