Roles & Responsibilities

• Coordinate and lead discharge planning to ensure safe, timely transitions from hospital to home or facility.
• Perform thorough biopsychosocial assessments to identify patient needs and resources.
• Facilitate care coordination with internal teams and external community partners to secure services and equipment.
• Document care, education, and plan-of-care clearly and promptly in Epic to support continuity and utilization review.
• Provide individualized patient and caregiver education to prepare patients for discharge and follow-up care.
• Manage appeals, prior authorizations, and benefits eligibility activities to support appropriate coverage.
• Advocate for patients with behavioral health needs, homelessness, substance use history, or complex social situations.
• Participate in family meetings, care conferences, and collaborative treatment planning to improve outcomes.

Qualifications

• Active CA state social work license required (pending licenses accepted).
• Minimum 2 years acute hospital case management or acute social work experience.
• Travel assignment experience required — this is a contract travel role.
• Proficiency with Epic and computerized documentation required.
• Comfort with every-other-weekend and holiday coverage; may be asked to watch up to two units on weekends.
• Ability to navigate community resources (Medicaid/Medicare, food assistance, transportation, DCF processes).
• Strong communication, interdisciplinary collaboration, and documentation skills.
• Ability to comply with RTO restriction (no more than 7 days).
• MSW preferred (listed as a preferred credential).

• Acute hospital case management and discharge planning expertise
• Biopsychosocial assessment and care coordination
• Epic/electronic charting and computerized documentation
• Knowledge of Medicare, Medicaid, prior authorization, and appeals processes
• Experience with behavioral health, 5150/involuntary holds, and substance use resources
• Comfort working with diverse age groups (neonates through geriatrics) and multidisciplinary teams
• Familiarity with home health, hospice, LTAC, rehab and long-term care referral processes
• Strong patient/caregiver education and advocacy skills
