Roles & Responsibilities

• Assess patients in their homes using professional nursing judgment to identify needs and risks
• Develop individualized care plans focused on oncology recovery, symptom management, and safety
• Implement and evaluate nursing interventions including infusion therapies when certified
• Administer chemotherapy/biotherapy and immunotherapy per protocols and certification requirements
• Provide leadership and supervision to CNA/SNA and other assigned healthcare staff
• Educate patients and caregivers on treatment, side-effect management, and home safety
• Document patient status, interventions, and outcomes accurately and timely
• Coordinate care with interdisciplinary teams to ensure continuity and quality of care

Qualifications

• Active Illinois Registered Nurse license (copy required with submission)
• Minimum 1 year of oncology nursing experience (required)
• Active Chemotherapy/Biotherapy Infusion and Immunotherapy certifications (required)
• ONS/ONCC certification (required)
• BLS (AHA) certification (required)
• Valid driver’s license and car insurance (required for home visits)
• COVID vaccination/status documentation required (declinations not approved)
• Supervisory experience and two supervisory references requested
• Current resume and active certifications/licenses at time of submission

Knowledge & Skills

• Strong patient assessment and clinical decision-making in oncology home care
• Skilled in care planning, implementation, and outcomes evaluation
• Experience with infusion therapy administration and monitoring
• Ability to lead, supervise, and mentor direct care staff
• Excellent patient and caregiver education skills
• Comfortable managing documentation and agency-required checklists
• Ability to travel locally to patient homes reliably and safely
