Roles & Responsibilities

• Provide safe, compassionate direct patient care including medication administration and treatments
• Assess patient condition, monitor vital signs, and promptly report changes to the care team
• Document patient care accurately and timely in the electronic health record
• Collaborate with physicians, therapists, and interdisciplinary staff to implement care plans
• Support patient mobility and activities of daily living while promoting dignity and independence
• Educate patients and families about care needs, discharge planning, and follow-up
• Maintain infection control standards and contribute to a safe clinical environment

Qualifications

• Current LPN or RN license in good standing (appropriate state licensure required)
• Completed an accredited LPN or RN nursing program
• Basic Life Support (BLS) certification is recommended or required by facility
• Comfortable working in post-acute/rehab settings and collaborating with interdisciplinary teams
• Professional, reliable, and patient-centered with strong communication skills

Education

• Graduation from an accredited LPN or RN program; high school diploma or GED required

Knowledge & Skills

• Safe medication administration and documentation practices
• Clinical assessment and patient monitoring
• Experience with electronic health record charting and documentation
• Strong communication and teamwork skills
• Time management and prioritization in a fast-paced care environment
• Basic wound care, ADL assistance, and restorative care principles
