Roles & Responsibilities
- Provide safe, compassionate direct patient care including medication administration and treatments
- Assess patient condition, monitor vital signs, and promptly report changes to the care team
- Document patient care accurately and timely in the electronic health record
- Collaborate with physicians, therapists, and interdisciplinary staff to implement care plans
- Support patient mobility and activities of daily living while promoting dignity and independence
- Educate patients and families about care needs, discharge planning, and follow-up
- Maintain infection control standards and contribute to a safe clinical environment
Qualifications
- Current LPN or RN license in good standing (appropriate state licensure required)
- Completed an accredited LPN or RN nursing program
- Basic Life Support (BLS) certification is recommended or required by facility
- Comfortable working in post-acute/rehab settings and collaborating with interdisciplinary teams
- Professional, reliable, and patient-centered with strong communication skills
Education
- Graduation from an accredited LPN or RN program; high school diploma or GED required
Knowledge & Skills
- Safe medication administration and documentation practices
- Clinical assessment and patient monitoring
- Experience with electronic health record charting and documentation
- Strong communication and teamwork skills
- Time management and prioritization in a fast-paced care environment
- Basic wound care, ADL assistance, and restorative care principles
