Roles & Responsibilities

• Provide compassionate bedside care to residents, ensuring safety, dignity, and comfort
• Administer medications and treatments accurately and document per facility policy
• Perform routine assessments and communicate changes in condition to the care team
• Collaborate with nurses, therapists, and providers to deliver coordinated care
• Educate patients and families about care plans, medications, and follow-up needs
• Maintain accurate, timely documentation in the electronic health record
• Support admissions, discharges, and transfers smoothly and professionally

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Previous LPN clinical experience preferred; new grads considered depending on facility needs
• Current BLS/CPR certification preferred or willing to obtain
• Comfortable working overnight shifts and functioning independently during your shift

Education

• Graduate of an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

• Strong clinical assessment and basic nursing skills including vital signs and wound care
• Medication administration and documentation accuracy
• Comfort with electronic health records and charting
• Excellent communication and teamwork skills
• Time management and prioritization in a fast-paced clinical environment
• Compassionate patient-centered care and cultural sensitivity
