Roles & Responsibilities

• Provide safe, compassionate nursing care to residents of the Hopkins Center as a Licensed Practical Nurse
• Administer medications and treatments accurately and document care in the patient record
• Perform routine assessments, report changes in condition, and escalate concerns promptly
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver coordinated care
• Support admissions, discharges, and transfers to ensure smooth patient transitions
• Assist with wound care, dressing changes, and other skilled treatments within LPN scope
• Maintain accurate, timely charting and follow facility policies and regulatory guidelines

Qualifications

• Current LPN license (practical nurse) — Pennsylvania licensure required or eligible
• Current BLS/CPR certification required
• Active immunizations and health screening as required by the facility
• Previous LPN experience in long-term care, skilled nursing, or similar settings preferred
• Strong clinical judgment, attention to detail, and a patient-centered approach
• Reliable, professional, and ready to represent the care team with compassion

Education

• Graduate of an accredited Practical Nursing program
• Completion of state LPN licensing requirements

Knowledge & Skills

• Solid knowledge of medication administration and documentation practices
• Comfortable with basic nursing assessments and reporting clinical changes
• Familiarity with electronic health records and accurate charting
• Wound care, infection control, and standard safety protocols
• Excellent communication skills for interacting with residents, families, and staff
• Ability to prioritize tasks and function well on night shift independently
