Roles & Responsibilities

• Provide skilled, patient-centered physical therapy to adults and/or pediatrics based on caseload
• Assess patient function and create individualized treatment plans to restore mobility and independence
• Deliver therapeutic interventions including exercise, manual therapy, modalities, and functional training
• Document evaluations, progress notes, and discharge plans clearly and timely
• Collaborate with nurses, physicians, and other therapists to coordinate high-quality care
• Educate patients and families on home programs, safety, and prevention strategies
• Participate in quality improvement, safety initiatives, and team meetings to enhance outcomes

Qualifications

• Licensed Physical Therapist (PT) — New Mexico licensure or ability to obtain licensure for this assignment
• Active clinical skills in evaluation, plan of care development, and therapeutic interventions
• Strong documentation skills and familiarity with common therapy documentation standards
• Excellent communication and teamwork skills to collaborate across disciplines
• Current Basic Life Support (BLS/CPR) certification preferred or obtainable

Education

• Doctor of Physical Therapy (DPT) preferred or entry-level degree in Physical Therapy (as required by state)

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and mobility restoration
• Comfortable with documentation, discharge planning, and outcome tracking
• Able to adapt treatment plans to meet individual patient goals and facility protocols
• Strong interpersonal skills for patient/family education and team collaboration
• Time management and caseload prioritization in a fast-paced clinical setting
